About The COACH Program®
Who is it for? Who can Join?
The COACH Program® telephone health coaching service is:
- FREE
- Run by specially trained health professionals
- Tailored to your lifestyle and you
- Based on evidence
You are eligible for the telephone health coaching services if you:
- Have type 2 diabetes
- Have pre diabetes
- Have previously been diagnosed with diabetes during pregnancy
- Or if you complete the AUSDRISK tool
(click here: www.bakeridi.edu.au/aus_diabetes_risk/ ) and score 12 or more.
How to join?
Joining The COACH Program® is quick and easy.
- Call us on 1300 745 153
- Our helpful staff will take a few details from you and book you in for your first coaching session.
- Then just sign and send back the consent form we post out to you in the reply paid envelope supplied.
- You might have also been referred to The COACH Program® by your GP or another health professional. Simply follow the same steps above and you can get underway.
